We are very pleased that you completed your tour and have decided to apply to Butler Montessori.

The online application that starts below is a seven-page form that will take approximately 15 minutes to complete. You can complete the form over several sessions. At any point, simply hit SAVE to store the information you enetered, and return later to continue.

Having the items listed here ready on hand will make completing the form go faster:

• Information about your child: name, age, current or previous schools.

• Household information for each home the child resides in: address, names and contact information for each parent or guardian that resides in that residence with the child.

• Sibling information: names, ages, where siblings attend school.

• Any special circumstances that we should be aware of: physical, emotional, educational, parental, as well as any provisions or testings that are pending or in place.

Once the online admissions application and $50 (per family) application fee is received, you will be contacted to schedule your admissions interview.
